Dublin Northeast Assertive Outreach and Case Management: Team Leader
TEAM LEADER

The Dublin Northeast Assertive Outreach and Case Management Initiative
is a new and exciting service, bringing together a number of addiction
services in Dublin Northeast. Its purpose being, to initiate a dynamic
service to support people with problematic drug and alcohol use and to
target those hardest to reach in Dublin **Apply on the website**.

TEAM LEADER POSITION

We are now recruiting a Team Leader to drive the initiative and to
manage a team of four assertive case management workers. The
successful candidate will lead the team in all aspects of outreach,
early intervention, and case management, targeting in particular
people most excluded and those not already linked into addiction
support and healthcare services.

As the Team Leader, you will be proactive in working with other
statutory, voluntary and community services, to provide a wide range
of supports as well as leading the Team to meet identified outcomes.

A key part of the role will be to capture and collate the information
required for the monitoring and evaluation of the effectiveness of the
initiative to support future funding applications.

THE SUCCESSFUL CANDIDATE WILL HAVE:

* Minimum of 3 years? experience working with in the areas of
addiction or related field.
* Experience of starting up and developing an initiative.
* Degree in addiction studies or related field, social care,
youth-work etc.
* Experience in leading and managing a team.
* Experience in undertaking assessments, developing care plans, and
case management within a framework similar to the NDRIC framework.
* Experience in evidence based frontline models such as SMART
recovery, CRA and MI etc.
* Significant experience in high quality interagency work and the
capacity to establish and build positive relationships with other
organisations.
* Direct experience of conducting programme evaluation and analysing
data to inform future service delivery
* Full clean driving license is essential
