ROLE TITLE: COMMUNITY REHABILITATION CASE WORKER

CONTRACT: Full-time, permanent (subject to probationary period)

REPORTING TO: Community Programmes Manager

ROLE SUMMARY:

* Facilitating clients to develop an individual goal plan and to
participate in activities within their own community.
* Providing information and support to clients and carers.
* Helping clients to overcome difficulties or barriers they
encounter in their day-to-day activities, increasing confidence and
independence through practical involvement.
* Working as a member of the Dublin Rehabilitation Team,
participating in providing support and services to existing clients,
including facilitating in-service groups and providing cover on the
Rehabilitation staffing calendar if required.
* Preparation and presentation of programmes appropriate to the
client group.
* Support of clients in the community in dealing with every day and
new activities without barriers, and in accessing supports in the
community.
* Presentations to a range of organisations and networking in the
community.

DUTIES AND RESPONSIBILITIES:

1.0 ASSESSMENT AND COMMUNITY SUPPORT
1.1 Assess the needs of clients in relation to community
reintegration.
1.2 Identify the client?s interests, capacities and needs.
1.3 Identify the type of activities preferred by and suited to the
client. Working closely with the client, and carer(s)/other
stakeholders if applicable, assess the client?s aspirations,
experiences, abilities, skills, knowledge and any potential obstacles.
1.4 Work with clients exiting RT programmes to develop and support a
weekly timetable of community activities.
1.5 Work with caseload of direct Community Rehabilitation referrals to
develop a specific strategy leading to community activities, in line
with individual client need.
1.6 Develop a network of disability friendly contacts for activities
and placements as needed.
1.7 Ensure that the client is an active participant in all phases of
the process.
1.8 Facilitate clients to take a lead in their own lifestyle choices
and to implement these to the best of their ability.
1.9 Identify potential suitable organisations by matching the needs of
the organisation to the needs, abilities and skills of the client.

2.0 ADMINISTRATION AND POLICY
2.1 In partnership with clients, develop and implement objective plans
and monthly updates.
2.2 Collect, collate and monitor data on clients? progress.
2.3 Record and share resources locally and nationally, as agreed with
Manger.
2.4 Facilitate and support the work of all Rehabilitation Services.
2.5 Maintain up to date documentation and reports in line with
departmental policies and procedures, keeping Manager updated at all
times.
2.6 Maintain client records to the required standard.
2.7 Undertake specific projects and administrative tasks as required
by manager.
2.8 Provide statistics and records of work as appropriate.
2.9 Keep up to date with all general administration duties as
required.

SKILLS AND EXPERIENCE:

* Degree level qualification in social care/rehabilitation or other
relevant discipline and substantial experience working with people
with disabilities. Experience working with individuals with ABI an
advantage but not essential.
* Full clean drivers? licence and access own car for work is an
essential requirement for this role.
* Experience of creating pathways to employment, education, housing
etc.
* Experience of case working/ case management
* Knowledge of community-based service and resources
* Group facilitation an advantage.
* Excellent communication and interpersonal skills.
* Reliability and a flexible approach.
* Excellent administrative, organisational and PC skills.
* Strong client centred, ethical approach.
* The ability to motivate and assist/support individuals in the
community.
* The ability to develop networks in the community and to act as
advocate for Headway and its clients.
* The ability to work both on own initiative and as part of a
multi-disciplinary team.
* Creativity (e.g., in using community resources, problem solving,
dealing with client needs).

on daytime office hours Monday to Friday (time in lieu granted for
evening/weekend as required).
* Permanent role with 6-month probationary period. You will be
