SAFETYNET PRIMARY CARE

The overall aim of Safetynet is the provision of quality services and
structures to enable access to primary healthcare and social inclusion
services for the homeless and other marginalised groups.

BACKGROUND ON SAFETYNET SERVICES

COMMUNITY ASSESSMENT HUB (CAH): Since May, Safetynet has been
operating a Community Assessment Hub adapted to the particular needs
of homeless and others living in extreme poverty and congregated
settings including those living in emergency accommodations including
Direct Provision and those with additional needs such as addiction and
those who don?t speak English. The hub provides assessment and
monitoring for those who are Covid positive or presumed positives. A
key function of the hub is the ongoing monitoring and follow up for
people in isolation with Covid **Apply on the website** at home or in
hostels.

The MOBILE HEALTH AND SCREENING UNIT (MHSU): The MHSU aims to add
capacity to current health services, identify and treating health
problems and reducing threats from communicable diseases. It aims to
facilitate integration of those on the margins (?hard to reach
groups?) to mainstream services. A large vehicle equipped as a
mobile medical clinic, (including a mobile X-ray and consultation
room) the service travels to various locations and collaborates with
existing service providers operating in each local area with a view to
integrating service users into local services.

INCLUSION HEALTH HUB (IHH): The Inclusion Health Hub in Summerhill is
for people who are not being served by other services including
undocumented migrants, Protection Applicants without access to primary
care services, as well homeless people. This clinic is one of the few
clinics in the homeless sector that sees children. There are allied
professionals from the HSE and other NGOs working at the Hub including
Physio Social worker addiction and homeless case workers. There is a
strong emphasis on interagency collaboration.

HOMELESS PRIMARY CARE TEAM (INREACH): This service is based in Dublin
in Supported Temporary Accommodations for people who are experiencing
homelessness. The approach is to deliver a clinic on site in homeless
facilities thereby bringing health services close to where people are.
This approach is based on the evidence that homeless people have
different health consulting behaviours than the housed population and
a very high burden of ill health. This service has been specifically
tailored to the needs of this vulnerable group.

MOBILE HEALTH UNIT OUTREACH (MHU): Using a converted ambulance, the
service physically travels to areas where rough sleepers have been
identified, bringing primary care to rough sleepers or other homeless
people without access to health services. The initiative links rough
sleepers to appropriate health services and removes health related
barriers to exiting

GENERAL JOB DESCRIPTION

The STAFF NURSE (GENERAL) will provide nursing services at Staff Nurse
level across all Safetynet teams:

* Community Assessment Hub (CAH)
* Mobile Health and Screening Unit (MHSU)
* Inclusion Health Hub (IHH)
* Homeless Primary Care Team (Inreach)
* Mobile Health Unit Outreach (MHU)

The position is based primarily in Dublin but with a national remit
involving travel to various locations as identified by HSE Social
Inclusion and Safetynet.

QUALIFICATIONS

* Candidates shall be registered in the General Division of the
Register of Nurses kept by An Bord Altranais or be entitled to be so
registered
AND
* Have at least 2 years post registration general nursing experience
AND
* Have adequate knowledge of general nursing administrative
procedures
AND
* Have the clinical, managerial and administrative capacity to
properly discharge the functions of the role
?
* Mandatory updates in CPR/Infection control/ Elder abuse/
Non-violent crisis intervention/ medication management
* Candidates shall be of good character.
* Candidates shall be in a state of health that indicates a
reasonable prospect of ability to provide regular and efficient
service.

DUTIES & RESPONSIBILITIES

The following sets out the duties and responsibilities of this
position:

_SERVICE DELIVERY_

* Support clinics in services providing support to GP
* Experience working with adults and minors
* Ability to autonomously conduct health assessments and infectious
disease screening with support of the clinical team
* Flexibility and ability to travel around Ireland which includes
overnight stays
* Adaptable working environments
* Under the clinical governance of respective service, provide
nursing services to patients attending the service e.g. cervical check
programmes, vaccination programmes, dressings and phlebotomy.
* Carry out services according to national guidelines in conjunction
with Safetynet?s policies and protocols
* Engage patients in health promotion and disease prevention
programmes
* Ability to work with translators

Cultural competence and awareness of intercultural challenges for
newly arrived migrants

_CLINICAL /PROFESSIONAL_

* Practice Nursing according to the Code of Professional Conduct as
laid down by the Nursing and Midwifery Board (An B?rd Altranais agus
Cnaimhseachais na Eireann) and Professional Clinical Guidelines;
* Work within scope of practice in conjunction with respective
service and take measures to develop and maintain the competence
necessary for professional practice;
* Maintain a high standard of professional behaviour and be
accountable for practice;
* Participate in innovation and change in the approach to patient
care delivery particularly in relation to new research findings,
evidence based practice and advances in treatment;
* Be aware of ethical policies and procedures which pertain to their
area of practice;
* Deliver the nursing care of an assigned group of patients within a
best practice / evidence based framework;
* Manage a designated caseload;
* Promote the health, welfare and social wellbeing of patients
within our services;
* Respect and maintain the privacy, dignity and confidentiality of
the patient;
* Ensure that care is carried out in an empathetic and ethical
manner and that the dignity and needs of the patient are respected;
* Provide appropriate and timely education and information to the
patient and be an advocate for the patient.

_ADMINISTRATIVE_

* Ensure that the care setting is maintained in good order using
appropriate models, that supplies are adequate and that all equipment
is clean and in good working order and ready for immediate use;
* Management of stock ordering processes;
* Ensure that equipment is safe to use and report any malfunctions
in a timely manner;
* Assist with ordering of supplies as required and ensure the
appropriate and efficient use of supplies is made and exercise economy
in the use of consumables;
* Maintain appropriate and accurate written and electronic nursing
records and reports regarding patient care in accordance with local /
national / professional guidelines;
* Provide clinical activity reports for Safetynet and funders;
* Liaising with other primary healthcare providers;
* Management and tracking of healthcare records and results;

_OTHER RESPONSIBILITIES_

* Undertake other responsibilities commensurate with the role
including administration as assigned by your Line Manager and/or the
CEO from time to time;
* Undertake the role in a professional manner maintaining a high
quality standard of work, and in accordance with the aims, values and
ethos of the Safetynet Primary Care;
* Adhere to Safetynet Primary Care policies and procedures;
* Be self-servicing in terms of word processing, filing, record
keeping, database input, administration, collating information and
generating reports from Safetynet Primary Care data etc;
* Participate in internal/external meetings as required;
* Be an effective member of the Safetynet Primary Care team and
participate fully in staff meetings and other committees of the
Safetynet Primary Care as required.
