ABOUT THE ROLE:

Family Carers Ireland wish to make the following appointment;

ROLE: Carer Wellbeing Manager

REPORTING TO: Head of Innovation & Strategic Partnerships

LOCATION: CORK/CHO 4

The role will require 1 day per week working in an outpatient
Gerontology Service in Cork supporting family carers as part of an
out-patient service and other days will be required to provide
services in acute hospitals / in the community in the Cork / Kerry
region as the project develops.
Where a physical presence is not required, the successful candidate
will work from home in line with Covid-**Apply on the website**

CONTRACT DURATION: One-year fixed term contract.

HOURS PER WEEK: **Apply on the website** per week. The normal working
hours are 9 am to 5.**Apply on the website** pm (Monday to Thursday)
and 9 am to 5 pm (Friday).

Family Carers Ireland has secured funding to deliver a new project ?
The Carer Wellbeing Project ? to family carers of older people in
CHO 4. The project involves triaging family carers of older people
based on risk and need, to one of three interventions: 1) Information,
Support and Guidance; 2) Carers Clinic and Caring with Confidence
Programme and 3) Intensive Wellbeing Review (using the Carers Star
assessment tool with follow-up every 2 weeks for **Apply on the
website** weeks).

The role of the Carer Wellbeing Manager in this project is to triage
family carers referred through the HSE, run the Carers Clinic (which
may be physical and/or virtual, deliver the Caring with Confidence
programme online and conduct intensive Wellbeing reviews. These
reviews use a strengths-based approach, require the Carer Wellbeing
Manager to provide a holistic assessment of the family carer?s need,
with a view to developing a person-centred action plan and
sign-posting to interventions to support a sustainable care, work
and/or education routine.

Utilising a ?resolution at first point of contact philosophy? and
knowledge of services and service providers, the Carer Wellbeing
Manager will connect the family carers to partners, external agencies,
in-house and on-line supports, including education programmes, respite
and/or counselling. In addition to direct individualised work with
family carers, the Carer Wellbeing Manager will specialise in a
specific area relevant to family carers (in this case, older persons).

DUTIES & RESPONSIBILITIES:

In this role, the successful candidate will be responsible for the
following;

_DELIVERY OF INDIVIDUAL SUPPORT TO FAMILY CARERS_

* Triage the family carers referred by the HSE using the agreed
triage tool.
* Set up and run a weekly online virtual carers clinic.

* Efficiently and effectively assess family carer needs for supports
using the Carers Outcome Star? in person (Covid-**Apply on the
website** dependent), via phone or video call depending on the
preferences of the Family Carer referred to the project.

* NB it is envisaged that most of this work will be done through
video call / phone due to Covid-**Apply on the website**.

* Develop and agree a person-centred action plan with the family
carer based on the Outcome Star? assessment, with SMART actions
agreed to support the family carer. Follow up with each carer every 2
weeks over a **Apply on the website**-week period, conducting a
mid-point review at 6 weeks and final assessment at **Apply on the
website** weeks.
* Assist family carers to access appropriate supports including
information, referral to other services, referral to online services
(including education), the emergency scheme, carer counselling,
respite, carers virtual clinic and education programmes.
* Advocate on the family carer?s behalf as required.
* Work with other Family Carers Ireland staff and external
stakeholders to ensure appropriate supports are put in place.
* Complete family carer follow-ups and reviews.
* Write case notes, goal and action orientated plans mapped against
outcomes.
* As required, liaise with family carers and negotiate with service
providers to put supports in place for high complex need situations.
* Act precisely and calmly when dealing with family carers in high
stress situations.
* Support family carers to complete an emergency care plan.
* Develop and use knowledge of services and supports offered
informally and formally internally and within the community to aid
family carers to access appropriate supports and information to
address their needs.
* Develop, understand and use knowledge of the challenges/stresses
facing carers to provide emotional support using a strengths-based
approach.

_SOURCE AND MANAGE REFERRALS_

* Liaise with external stakeholders to explain the project and to
source referrals.
* Engage with media to promote the project.
* Support a Gerontologist with family carer issues and source
referrals through a joint gerontology / carers? clinic in an acute
hospital out-patient department on a weekly basis in Cork (near
Douglas).
* Source referrals through the HSE?s Winter Initiative in CHO 4.

_DEVELOP WITHIN AN AGREED SPECIALIST AREA_

* Depending on your own experience, preference and considering the
needs of the organisation develop your expertise in an agreed area ?
in this project the need is to specialise in the subject of caring for
an older person.

_EDUCATION_

* Deliver the Caring with Confidence Programme to family carers and
other education and information sessions as required.

* This may be online due to Covid-**Apply on the website**.

_PERFORMANCE MANAGEMENT_

* Carry out your position and responsibilities in line with the
Organisation?s values, policies, procedures and processes.
* Undertake all reasonable work instructions in a timely and
professional manner.

communications, projects and other workplace initiatives.

carer conversations.
* Attend team meetings and events.

_REPORTING_

* Adhere to all internal procedures in spending and controlling the
Organisation?s funds.
* Maintain records of contacts with family carers, engagements with
staff and other records required by the organisation as set out in
organisational policies, procedures and guidelines.
* This role will require use of the Organisation?s CRM and Carers
Outcome Star systems online.
* Provide progress reports as required ? both internal and to
funders.
* Keep detailed case notes associated with each family carer who
engages with you.
* Engage in activities to monitor / evaluate the service.
* Submit reports as required to inform business plans and the annual
report.

_HEALTH SAFETY AND ENVIRONMENTAL_

* Ensure the Organisation?s Health and Safety policy and procedures
are adhered to and carry out your roles and responsibilities as
detailed in the relevant policies and procedures.
* Report and engage in the investigation of any safeguarding issues,
Children First concerns or complaints as per the Organisation?s
procedures.

* Undertake other duties as may be required and assigned by the
Organisation from time to time.

ROLE CRITERIA:

The ideal candidate will have the following qualifications, skills and
attributes:

* A degree (QQI 7 or higher) in the area of Social Work / Community
Development / Social Care / Psychology / Supported Employment or
healthcare related area;
OR
* A minimum of five years? experience working with families in
similar situations;
* Previous experience providing support to families or carers
(ideally relevant to older persons context);
* Knowledge of challenges facing carers;
* Ability to prioritise tasks and work within a dynamic environment;
* Experience of working remotely;
* Excellent IT Skills - mainly MS Word, Excel, Outlook, PowerPoint
and experience and confidence using videoconferencing platforms e.g.
Zoom;
* Proven networking skills;
* Flexibility in attitude and approach to the job;
* Reflective approach to their work;
* A strong work ethic;
* Willingness to do some evening work when required;
* Strong facilitation and presentation skills;
* Excellent oral, empathic communication skills and the ability to
establish rapport with a diverse range of people;
* Demonstrated ability to work under pressure, make clear and quick
decisions and work with clients in distress;
* Effective time management;
* Full, clean drivers licence and access to own transport;
* Ability to work autonomously and within a team.

The following is also desirable:

* Experience working with and responding to client?s individual
needs which may include advocacy;
* Experience negotiating with community-based organisations or
groups to maximise outcome for clients;
* Demonstrated ability to apply a person centred or strengths-based
approach and a consumer directed model of service provision;
* Knowledge of services and supports offered formally and informally
within the community services sector.
