1. ROLE

Depaul is a values-led organisation working with a number of services
throughout Dublin and Belfast. This role will provide on-site nursing
support to staff and service users around health related issues and
supervision of Health Care Assistants. Based at our flagship service,
Sundial House, the post holder will help lead our health team at the
service.

2. KEY AREAS OF RESPONSIBILITY

* Under the Health Initiative, maintaining a high standard of nursing
care at Sundial house and any other Depaul service that the
post-holder will be working in, by providing skilled nursing
assessment and intervention around Chronic Disease Management.
* To develop clinical expertise through nursing practice and
multidisciplinary working, to support individuals with complex health
needs
* To maintain a high standard of professional and ethical
responsibility. Working within the scope of nursing practice as
specified by NMBI to develop Clinical Governance and care standards
specific to the needs of the setting.
* To support the staff team around health related care planning and
maintaining a high standard of care including ongoing review, referral
and discharge planning, hospital liaison and management of GP clinic.
* To advocate, refer and co-ordinate appropriate mainstream health
services e.g. Primary care teams, community and specialist mental
health services, Allied health services and inpatient/outpatient
services.
* To support referral to treatment services including alcohol detox,
drug stabilisation, Intermediate Care/Respite and completion of
related paperwork in conjunction with keyworker and GP.
* To actively participate development and implementation of best
practice in line with Clinical Governance with regards to homeless
health service delivery.
* To prepare and update nursing specific care plans in conjunction
with the Clinical Nurse Manager and local management team. To oversee
and review personal care provision provided by Health Care Assistants.
* Commitment to continuing professional development, quality
standards and empowering others
* Promoting and assisting with Health Promotion, education and
Screening events: seasonal flu and vaccination programmes, Blood borne
virus/STI testing, TB screening and other National Screening Services
* Assisting and supporting service users to engage with community
and hospital based appointments including on site GP service where
required
* Provision of general Nursing interventions and assessments
including wound care, Chronic Disease management, support with care
planning for self-management, nutrition, medication management,
mobility, frailty and cognitive assessments etc.
* Discharge planning and anticipatory care planning around
resettlement, long term care and end of life
* Close liaison with the Inclusion Health Services and participation
in development and progression of best practice and research
initiatives.
* To maintain open communication with staff using localized
communications protocols ie. Handover, incident review
* To observe Depaul Health & Safety Procedures in particular regular
review of activity risk assessments.
* To observe clear professional boundaries in relations with the
service users we work with.
* To respect service users right to confidentiality in our
relationship with them as per the Depaul Confidentiality policy and
Code of Practice.
* To line manage a team of Health Care Assistants providing
supervisions and JARs.

The above list is not exhaustive; additional areas of responsibility
may be added over time and flexibility to cover for other staff roles
is required from time to time.

3. PERSON REQUIREMENTS

* Have at least 1 years? experience post registration working in
the community, primary care or residential setting desirable.
* Have active registration with NMBI.
* Have a good standard of education. Have an understanding of harm
reduction and low threshold working.
* Have an understanding of health issues related to homelessness and
the complexities they may present.
* Understanding of risk management principles and infection control.
* Capable of responding calmly in a crisis and use your own
initiative to deal with incidents as they arise
* Be able to work as part of a team understanding the importance of
team communication.
* Have an understanding of Health and Safety and the health and
safety requirements within a residential setting working.
* Have a knowledge and understanding of the vision, mission and
values of Depaul.
