JOB TITLE: HEAD OF PATIENT SUPPORT

The Irish Heart Foundation is Ireland?s national charity dedicated
to fighting heart disease and stroke. Every hour someone in Ireland
suffers a stroke and every day hundreds of people are diagnosed with
heart conditions. The lives of these people are often cut tragically
short. Many are left with disabilities. Almost 9,**Apply on the

making cardiovascular disease one of the nation?s biggest killers.
We work to turn this around ? support people to live healthy lives
and avoid cardiovascular disease, but also to support people living
with heart conditions and stroke.

THE ROLE

Our support services have grown rapidly in recent months in response
to massive unmet need among stroke survivors and people living with
heart conditions. Their purpose is to enable everyone affected by
cardiovascular disease to live the fullest and healthiest lives
possible in the community.

We are now seeking a social care professional with the experience and
drive to enable our patient services achieve the next stage of their
development, building sustainable structures, maintaining service
excellence, measuring impact and securing statutory funding that will
ensure high grade heart and stroke support is available countrywide.

In addition to expanding existing services, we intend developing major
new digitally based support offerings and a dynamic volunteer
programme, together with fostering closer relationships with statutory
health services.

KEY RESPONSIBILITIES

* Leading the operational management and development of the
Foundation?s stroke, heart failure and other heart support services.
* Securing core funding for key services through persuasive
proposals to health services at local and national level; and the
development of strong relationships with key decision makers and
influencers in health and social care commissioning and delivery.
* Overseeing the continuing development of strong external
relationships across the national cardiovascular care community,
ensuring there is broad-based knowledge, understanding and support for
the services we provide.
* Working with key staff to develop service structures, with
particular emphasis on a volunteer recruitment, management and
training programme.
* Developing effective monitoring and measurement systems that
demonstrate the effectiveness of our service delivery and its impact
on patient experience and outcomes.
* Developing and implementing comprehensive risk management and
mitigation plans and ensuring the delivery of timely, safe, high
quality services that comply with all relevant regulation, care
standards and legal responsibilities
* Creating and implementing quality assurance and continuous
improvement programmes ensuring the highest standards across all our
services
* Monitoring and benchmarking the Foundation?s services against
new developments and international best practice in heart and stroke
support
* Creation, ownership and management of delegated aspects of the
patient support budget and financial delivery, deploying resources
where they are most required to provide high quality stroke support.

PERSON SPECIFICATION

* At least five years? experience of developing and delivering
support services for people living with long-term conditions,
including the development of volunteer support structures.
* Educated to degree level in a relevant discipline, or equivalent
relevant and demonstrable professional experience
* Excellent understanding of health and social care commissioning
and delivery in Ireland
* Experience of overarching responsibility for development of
written proposals, business cases and tendering processes; and a track
record of securing significant levels of statutory funding.
* The ability to be proactive and flexible, with the skills to coach
and lead a diverse team.
* Experience of developing and managing strong relations and
partnerships with external organisations, bodies and stakeholders.
* A track record of implementing quality improvement systems that
deliver consistently high standards for service users.
* The ability to implement robust impact evaluation measures and to
use these effectively to promote service development.
* Expertise in service delivery on digital platforms and its
potential to provide services to more people.
* Evidence of continuous professional development.
* Experience of financial managing and budgetary monitoring

_DETAILS OF ROLE AND APPLICATION PROCESS_

_This is a full-time role, Monday to Friday for a one year fixed term
contract. The role is based in the Irish Heart Foundation?s offices
in Rathmines, Dublin. Currently staff are working remotely and the
successful candidate will be supported to work remotely initially
according to public health guidelines._
