POSITION TITLE: CLAN PROJECT COORDINATOR

AGENCY/PROGRAMME: Crosscare Homeless Services

LOCATION: Crosscare Arran Quay

WORK SCHEDULE: Full Time ? 3 year Fixed Term Contract ?(**Apply on

REPORTS TO: Manager, Amiens St

ROLE PURPOSE:

To lead the CLAN team with the aim to support people moving from
Homeless services into independent living. To provide and sign post
supports to those suffering from social exclusion issues in particular
mental health/wellbeing and isolation. To promote peer led approach to
tenancy sustainment and develop the service to support this. Secondly,
to support Peer Mentor volunteers to develop skills and achieve their
own personal goals around education and career.

THE ROLE:

_GENERAL RESPONSIBILITIES:_

* Support the CLAN team of Support worker and peer volunteers to
provide housing related support to adults moving out of homelessness
e.g. social welfare, connection with the community and building
support networks.
* Engage with and build professional trusting relationships with
individual service users, treating them with dignity and respect, and
working within the values of Crosscare.
* Sign post to specialist support to residents in relation to mental
health issues
* Build and develop links with mental health teams, Local GP?s,
Pharmacies & other relevant support services for clients.
* Provide interventions to individuals and groups that promote
positive mental health within the service.
* Assist residents to access relevant outside services. Make
referrals to appropriate services. Accompany service users to
appointments & where necessary make visits, this may include hospital,
prisons, courts etc.
* Prepare monthly reports on mental health & our client group,
identifying trends, gaps, blocks etc. and present to Crosscare
Management and external forums as appropriate.
* Assist in Management in the development of policies & procedures
that are responsive to the needs of service users with mental health
issues, both diagnosed & undiagnosed. To liaise with and advocate to a
broad range of external service providers in addition to internal
services.
* Case management of the CLAN clients from referral stage through
assessment, development of individual plans, matching with peer
volunteers and connections within the community.
* Supervision of the CLAN team including volunteers and students.
* Development of peer mentor volunteers through training, goal
setting, coaching and mentoring.
* Sourcing and managing funding applications from external funders
for CLAN and the Community College and implement programs in line with
funding requirements
* Maintaining current programs and developing new programs within
CLAN eg: One Step Beyond, Social Club, Monday Morning Group.
* Develop and maintain relationships with other support services in
the community.
* To support Service User Involvement activities in Homeless
Services.
* To promote CLAN in Homeless Services by attending team meetings,
resident meetings and managers meetings.
* Recruitment and training of Volunteers and staff.
* To represent Crosscare at external meetings.
* To keep detailed, comprehensive records including outcomes, costs
and spending and report back to funders.
* To work within the ethos and values of Crosscare
* To attend training relevant to the work
* To carry out any other duties assigned by your line manager

THE HOLDER:
(QUALIFICATION/SKILLS REQUIREMENTS)

_REQUIREMENTS:_

* At least 1 year Team Leader experience or 3 years Keyworking
experience
* A relevant QQI Level 7 Qualification: Social Care, Social Science,
Community Development, Mental health, Psychology, Counselling?
* Full, Clean Driving Licence.

PLEASE NOTE: GIVEN THE NATURE OF THIS ROLE AND THE VULNERABLE SERVICE
USERS ACCESSING OUR HOMELESS SERVICES, ALL PERMANENT AND RELIEF STAFF
WORKING WITHIN THE SERVICE WILL BE REQUIRED TO BE FULLY VACCINATED
