This belief is embedded in our care pathway for residents in our
service
DUTIES WILL INCLUDE
* Deliver high quality, safe, effective, person-centred care and
services to all residents
* Ensure all Family Integration Plans (FIP?s) are developed,
implemented, updated and evaluated to meet the needs of the individual
residents and assisting residents in ensuring that the goals and
objectives are met in accordance with their needs
* Establish and maintain relationships with residents based on
respect and equality
* Support residents to integrate into their new communities ?
register them with GP?s, find schools for children, support them to
enrol in english language classes, to obtain their PPSN numbers and
medical cards
* Support residents to complete activities of daily living and
identify social clubs aligned to their interests e.g. rugby, football,
art, dance, drama etc.
* Empower residents to exercise their autonomy and independence,
including proactively supporting their independence through assisting
their ongoing education, training and employment
* Complete key working duties as assigned by the Social Care Leader
* Engage in professional supervision with the Social Care Leader and
MDT team as required
* Participate in the development and implementation of D?dean?s
Quality and Safety Management System (incident, accident, risk and
complaints management, audits, quality initiatives etc.)
* Maintain a safe environment for residents, staff and visitors
* Participate as a team member of the multi-disciplinary care team
in all aspects in the delivery of resident care
* Be aware of the confidential nature of the work and in particular,
the right to resident?s confidentiality. To this end, D?dean?s
residents shall not be discussed with people outside of D?dean
* Adhere to the Safety, Health and Welfare at work policy of the
organisation
* Keep up to date with D?dean?s policies and procedures and
guidelines and that these are adhered to at all times
* Undertake such other duties as may be assigned from time to time
by the Social Care Leader
* Have a positive ?can do attitude? towards your work at all
times and have a high level of ownership of your area of
responsibility within your work
* Display initiative by thinking of and recommending suitable
improvements to programmes
* Maintain written procedural records and make verbal reports on
each resident to ensure that the appropriate handover between the
individual staff members is carried out
* Maintain all such files and records appropriate to the area of
work as deemed necessary by management

REQUIREMENTS
* A Third Level Qualification in Applied Social Studies in Social
Care or equivalent qualification that is relevant to the care industry
is essential for the Social Care Worker Roles.
* FETAC Level 5 in Healthcare or equivalent?is essential for the
Assistant Support Roles
* Knowledge of legislation that guides social care
* Full, Clean Driver?s License
* Prior experience in a similar role
* Experience in caring for people
* Experience of report writing and personal support plans
