JOB TITLE: STROKE CONNECT SERVICE COORDINATOR

CONTRACT: FULL TIME, PERMANENT

LOCATION: RATHMINES OFFICE AND WORKING FROM HOME (HYBRID)

The Irish Heart Foundation is Ireland?s national charity dedicated
to fighting heart disease and stroke. Every hour someone in Ireland
suffers a stroke and every day hundreds of people are diagnosed with
heart conditions. The lives of these people are often cut tragically
short. Many are left disabled. Almost 9,**Apply on the website** die

cardiovascular disease one of the nation?s biggest killers. We work
to turn this around, and support people to live healthy lives and
avoid cardiovascular disease, but also to support people living with
heart conditions and stroke.

Returning home from hospital can be daunting for many stroke
survivors. To help with the transition, the Irish Heart Foundation
provides a dedicated phone service to support and empower stroke
survivors after discharge from hospital ? Stroke Connect Service.
People joining the service (becoming IHF members), receive eight weeks
of phone support from our team of experienced coordinators. Members
may have an additional eight weeks of support from a trained
volunteer. Following Stroke Connect, members have a variety of
long-term and short-term support options available to them.

We require a person with excellent organisational and interpersonal
skills to coordinate this vital service. The successful candidate will
have strong verbal and written communication skills and an ability to
interact effectively with stroke care providers and stroke survivors.

THE ROLE

This role offers an exciting opportunity to an experienced
administrator to work in the IHF?s Stroke Connect Service. This is
an evolving service and the role will develop alongside this service
evolution. The role involves working within the patient support team
to coordinate the administration and operational functions of the
Stroke Connect Service, under the direction of the stroke services
manager. It includes facilitating the intake of stroke survivors into
the service, managing their journey through the service, and
coordinating staff to provide practical support and information to
service users by phone after their discharge from hospital.

KEY RESPONSIBILITIES

* Log new referrals on the IHF?s database, screen referral forms
and escalate incomplete or queried referrals.
* Schedule admin calls with new referrals to ascertain consent and
check contact details.
* Schedule Needs Assessments with IHF support line nurses.
* Assign new referrals to stroke support coordinators and schedule
first call dates.
* Communicate efficiently between referring hospitals and other
healthcare settings, stroke coordinators, volunteers, support line
nurses, patient support team members and other contacts.
* Escalate any issues which may arise on calls to the Stroke
Services Manager as required.
* Schedule regular SCS Steering group meetings, set agenda and keep
minutes.
* Ensure stroke support coordinators updates Needs Assessment at the
end of SCS service and ensure further support options for members are
logged for the next stage of service delivery.
* Monitor and oversight for referring members into parallel
services. Coordinate members into these services ? e.g. Professional
Counselling.
* Provide IT and technical support to stroke coordinators on the
IHF?s database (CRM/Patient Management System) and SharePoint
system.
* Coordinate the training schedules for the new CRM/Patient
Management System.
* Promote the service within the HSE and other settings.
* Survey and evaluate the impact of Stroke Connect.
* Participate in the patient support policy group, contributing to
the ongoing review and development of policies and procedures
associated with Stroke Connect.
* Input into digital, social media and print content in relation to
information sharing and the promotion of Stroke Connect.
* Working with the Stroke Services Manager and other team members,
monitor and schedule Facebook posts in the IHF private Facebook groups
for Stroke Survivors and Carers.
* Updating the CRM/Patient Management System and Learning Management
System as required.

_REPORTS:_

* Monitor and report on weekly calls made to all stroke survivors and
their families.
* Working with the Stroke Services Manager, coordinate monthly
reports on service activity and produce a quarterly stakeholders
report.

SKILLS REQUIRED

* Strong IT, database and MS Office skills are essential.
* Experience in the implementation of a CRM/Patient Management
System and a Learning Management System is desirable, but not
essential.
* A self-motivated person with strong administration, communication
and time management skills.
* Strong communication (verbal and written) and interpersonal
skills.
* Experience coordinating and scheduling service entry for service
users.
* Ability to build professional relationships internally and
externally
* Strong planning capabilities are essential.
* Ability to manage competing deadlines
* Experience of patient support settings and knowledge of stroke an
advantage
* Excellent attention to detail.
* Fluent written and spoken English.
* Ability to represent the Irish Heart Foundation in a professional
manner at all times.

DETAILS OF ROLE AND APPLICATION PROCESS

This is a full-time, permanent role, Monday to Friday. The role
requires attendance in the Irish Heart Foundation?s offices in
Rathmines, Dublin, two days per week (currently). Staff may attend the
office or work remotely on the other three days.
