INITIATIVE ? ST MARYS CAMPUS - COMMUNITY CONNECTOR FOR OLDER PERSONS

Gabrielle O?Keeffe the Head of Service, Health and Wellbeing, Cork &
Kerry is inviting expressions of interest from organisations with a
track record of working with older persons to deliver programmes to
improve their health and wellbeing to roll out a Community Connector
service aligned to the social prescribing service. This will involve a
partnership approach between the Community and Voluntary sector and
the Health Sector (HSE and General Practice) to connect older adults
referred from community based Integrated Care Teams for Older People
with sources of support within their community. Previous experience in
delivering a social prescribing project is not essential.

The ICPOP Community Connector service is a specialist social
prescribing service for a defined caseload of older people (across 3
CHN?s in this instance this post will be servicing north side hub)
referred from a single source ? an Integrated Care Team for Older
People. (The Community Connector will adopt a coaching role with a
defined caseload of older people known to older person integrated care
services locally (e.g. seen by and optimised by Community Specialist
Older Persons? Teams/ Front Door Frailty Teams and now managed by
Primary Care Teams).

Organisations selected to deliver the Community Connector Service as
part of the Healthy Communities Initiative will be funded to deliver a
contracted level of service in line with the existing S**Apply on the
website** Health Act **Apply on the website** Service Arrangement with
HSE, and will include:

* Roll out of a Community Connector service in partnership with the
HSE and other key stakeholders( especially Health Action Zone
Community Health Workers and Social Prescribing link workers in the
area) targeting Older Persons living who access the Integrated Care
Programme for Older Persons
* Recruitment, employment and line management of a Community
Connector
* Provision of administration/technical support e.g., IT equipment,
confidential client data storage etc
* Ensuring required monitoring and evaluation processes are in place
* Responding to gaps identified through social prescribing
* Completion of reports required under the contractual agreement
with the HSE
* Financial reporting and management of budget provided
* Being supplied with relevant programme materials and resources to
support the delivery of prescribing programme from the HSE which will
also include core training for Social Prescribing Link Workers
* Facilitation of the Community Connector in the MDT and ensure they
are facilitated and ensure they are facilitated to attend MDT meetings
with the ICPOP.

THE HSE WILL PROVIDE THE FOLLOWING SUPPORTS:

* Funding to provide an agreed quantum of service including a full
time Community Connector worker, recruitment, office and
administration costs
* A social prescribing framework to guide development of local
service model
* Essential skill training for a Community Connector
* Monitoring and evaluation tools.
