JOB TITLE: STROKE CHECK IN SERVICE COORDINATOR

MANAGER: PATIENT SUPPORT MANAGER

The Irish Heart Foundation is Ireland?s national charity dedicated
to fighting heart disease and stroke. Every hour someone in Ireland
suffers a stroke and every day hundreds of people are diagnosed with
heart conditions. The lives of these people are often cut tragically
short. Many are left disabled. Almost 9,**Apply on the website** die

cardiovascular disease one of the nation?s biggest killers. We work
to turn this around ? support people to live healthy lives and avoid
cardiovascular disease, but also to support people living with heart
conditions and stroke.

Returning home from hospital can be daunting for many stroke
survivors. To help with the transition, the Irish Heart Foundation is
working with the HSE to provide crucial support via a dedicated phone
service providing regular calls to stroke survivors after discharge
from hospital.

We require a person with excellent organisational and interpersonal
skills to coordinate this vital service. The successful candidate will
have strong verbal and written communication skills and an ability to
interact effectively with stroke care providers and stroke survivors.

THE ROLE

The role involves working within the Irish Heart Foundation?s
patient support team to run our stroke check-in service, coordinating
staff and volunteers to provide practical support and information to
stroke survivors by phone after their discharge from hospital.

KEY RESPONSIBILITIES

* Connect stroke survivors referred from hospitals and other settings
to appropriate stroke group coordinators and volunteers.
* Communicate efficiently between hospitals, stroke group
coordinators, volunteers, support line nurses, patient support team
members and other contacts.
* Monitor and report on weekly calls made to all stroke survivors
and their families.
* Support stroke group coordinators and volunteers in relation to
any issues which may arise on calls and escalate to relevant team
members or other individuals if necessary.
* Coordinate further support options at twelve weeks for all stroke
survivors.
* Identify and develop additional support options within the service
for stroke survivors and their families.
* Provide training, information and support sessions to volunteers.
* Provide IT and technical support to volunteers and stroke group
coordinators.
* Promote the service within the HSE and other settings.
* Survey and evaluate impact of the Stroke Check In Service.

SKILLS REQUIRED

* A self-motivated person with strong administration, communication
and time management skills.
* Strong communication (verbal and written) and interpersonal
skills.
* Ability to build professional relationships internally and
externally
* Strong planning capabilities are essential.
* Ability to manage competing deadlines
* Experience of patient support settings and knowledge of stroke an
advantage
* Excellent attention to detail.
* Fluent written and spoken English.
* Strong computer and MS Office skills.
* Ability to represent the Irish Heart Foundation in a professional
manner at all times.
* Ideally candidates will be educated to third level with a relevant
qualification.

_DETAILS OF ROLE AND APPLICATION PROCESS_

_This is a full-time role, Monday to Friday for a one year fixed term
contract. The role is based in the Irish Heart Foundation?s offices
in Rathmines, Dublin. Currently staff are working remotely and the
successful candidate will be supported to work remotely initially
according to public health guidelines._
